"Swavalamban" is a prestigious health insurance scheme launched as part of a joint venture initiative by Govt. of India along with The New India Assurance Company limited providing an insurance coverage upto Rs 2 lakhs/- for Differently abled persons belonging to BPL category. The scheme aims to provide affordable Health Insurance to persons with Blindness, Low Vision, Leprosy cured, Hearing Impairment, Loco-Motor disability, Mental-Retardation and mental illness. The Scheme is co-ordinated by the State Initiative on Disabilities (SID), a project of Kerala Social Security Mission for Department of Social Justice. 

Guidelines for enrolling:-

• Persons with Blindness, Low Vision, Leprosy cured, Hearing Impairment, Loco-Motor disability, Mental-Retardation and mental illness can apply.

• Applicants between age limit 0 - 65 years belonging to BPL category (40% disability or more)

• Combined Annual Income of the family should not exceed Rs. 3 lakhs/- 

• Premium contribution of Rs. 357 per person

• Copy of valid ID i.e. Voter card, Aadhar Card, Driving license etc.

• Family size up to 1+3. Primary member is Differently abled. In case of minor being PWD, parent/legal guardian of such   minor can also be covered under the scheme.

• Passport size two photographs of each family member to be covered in the policy.

• Disability medical certificate as per PWD Act 1995

• Self attested copy of Ration card/ BPL/Priority category/ AAY (Antyodaya Anna Yojana)

How to apply-

Duly filled application forms have to be submitted to the concerned ICDS project offices along with supporting documents in the prescribed form. The Application forms are available in the websites of Kerala Social Security Mission and Social Justice Department.
